In order to process your registration in a timely manner, please complete the New Patient Questionnaire and hand-carry it to your scheduled appointment.
Arrive 15 min early with the following items:

• Completed Questionnaire
• Heath Insurance card/cards
• Valid identification card
• Insurance authorization form (if applicable)
• Copayment (if applicable)

Additionally, you may be asked to provide a hard copy of the following records:
• Radiology images on a portable CD
• Paper copies of any laboratory data and radiology reports pertinent to your consultation
• Signed record release (if applicable)

Please note:
Our office strictly enforces a minimum 24 hr cancellation policy. Be advised any appointment not cancelled within that time frame will be subject to a $50.00 per occurrence fee.
